Let’s get your account set up.

This onboarding form helps us collect the information needed to create your profile, understand your organization, and complete your membership or program setup.

The form will guide you through a few short sections and should only take a few minutes to complete.

Revenue or current operating budget information
Current accreditation status, if applicable
A valid credit or debit card for payment setup

What We’ll Ask For
1. Personal Information

We’ll ask for your name, preferred name, email, ethnicity, and individual role or segmentation.

This helps us identify the primary contact and better understand who is participating in the program.

2. Organization Information

We’ll collect details such as your organization name, job title, address, counties served, mission statement, service sectors, communities served, organization type, staffing numbers, revenue or budget details, and accreditation status.

This helps us understand your organization’s structure, service area, and eligibility details.

3. Payment Information

A valid credit or debit card is required to complete the setup process.

Payment information is used for authorized membership, program, or onboarding-related charges. Your payment is securely processed by our processor Stripe and no card data is stored on our servers.

Your information will be used for onboarding, account setup, communication, reporting, and billing purposes. Please ensure all required fields are completed accurately before submitting the form.
