Your personal contact information
Your organization’s contact details and address
Your organization’s mission statement
Counties and communities served
Staffing information
Revenue or current operating budget information
Current accreditation status, if applicable
A valid credit or debit card for payment setup
We’ll ask for your name, preferred name, email, ethnicity, and individual role or segmentation.
This helps us identify the primary contact and better understand who is participating in the program.
We’ll collect details such as your organization name, job title, address, counties served, mission statement, service sectors, communities served, organization type, staffing numbers, revenue or budget details, and accreditation status.
This helps us understand your organization’s structure, service area, and eligibility details.
A valid credit or debit card is required to complete the setup process.
Payment information is used for authorized membership, program, or onboarding-related charges. Your payment is securely processed by our processor Stripe and no card data is stored on our servers.
